Formidable Forms
AffiliateWP records a referral when a visitor an affiliate sent submits your Formidable Forms form, worked out on the amount field you pick.
Referrals From Your Formidable Forms
Each form gets an AffiliateWP section in its settings. Pick the field that describes the referral and the field that holds the amount, and each submission from a visitor who came through an affiliate’s link records a referral for that affiliate.
A referral with an amount waits as pending until Formidable reports the payment complete. Refund the payment later, and AffiliateWP rejects the referral if you haven’t paid it yet.
How It Works
Turn on Formidable Pro
Check Formidable Pro under AffiliateWP › Settings › Integrations. It covers Formidable Forms too.
Pick the form’s fields
In the form’s settings, open AffiliateWP and choose a Referral description field and a Purchase Amount field.
Referred submissions earn
A visitor who arrived through an affiliate’s link submits the form, and AffiliateWP records the referral.
What You Can Set
Referral description
Use any field on the form as the referral’s description, such as a product or a plan name.
Purchase Amount
The referral is your rate applied to the value in this field. A field with no amount records a 0.00 referral on a percentage rate.
Referral Type
Record each form’s referrals as a Sale, an Opt-In or a Lead.
Payments decide the status
A completed payment marks the referral unpaid, and a refund rejects it if you haven’t paid it yet.

That’s its name under AffiliateWP › Settings › Integrations. It works with Formidable Forms, and it doesn’t check for the Pro version.
Yes. Pick any field as the Purchase Amount. With a flat referral rate the affiliate earns that amount, and with a percentage rate the referral is 0.00. Either way it’s marked unpaid straight away.
When Formidable marks a completed payment refunded, AffiliateWP rejects the referral if you haven’t paid it yet, and adds a note to the entry. A referral still pending stays pending.
